Client: Central New York Care Collaborative (CNYCC, or CNY Cares) is a PPS with an extensive network across upstate and central New York with more than 2,000 healthcare and community based service providers. CNY Cares’ goal is to serve the population by improving the coordination of healthcare services, enhancing the quality of performance outcomes, and creating an overall better system of care for patients.

Problem: Like other newly formed PPSs, CNYCC needed a streamlined, centralized solution for delivering its projects and initiatives. A priority early on was to provide workforce training across its network and collect workforce data in one centralized database. CNYCC was also struggling to efficiently deliver and manage workforce blended workforce trainings that included online, classroom, and webinar learning.

Additionally, CNYCC needed a solution for managing workforce data submitted by partners.

HWNY Solutions: HWNY deployed its customized Learning Management System built around DSRIP deliverable needs to allow CNYCC to manage blended learning and track partner workforce reporting data.

To ensure all workforce training information could be aggregated in one central location on HWapps, HWNY custom developed reporting tools for partners to submit trainings they delivered outside of the LMS. Partners could then easily report to CNYCC key DSRIP metrics through HWapps for trainings they were self-managing.

In addition, as a partner, HWNY provides DSRIP workforce consulting and partner support and engagement to ensure that CNYCC’s DSRIP deliverables can be met. HWNY provides partner outreach, engagement, and support to educate on workforce reporting requirements and processes.

Impact: HWNY’s technology and services have allowed CNYCC to be successful throughout DSRIP and successfully begin retraining their network in a more value-based direction.
